Inpatient Referrals
CALL US FIRST at 833-227-2687 to discuss patient needs for inpatient hospitalization and relevant information. We will ask for necessary demographics as well as behavioral and medical history to determine the next steps.
We welcome walk-ins and may be able to help avoid an unnecessary trip to the emergency room.
If necessary due to medical history or history of substance use, or if the potential patient has self-harmed or attempted suicide, we may recommend sending the potential patient to the nearest hospital emergency room for medical clearance and treatment prior to a transfer to Arbour hospital.
Information we may ask for or request that it be sent or brought to us at time of referral:
- Patient’s full name
- Address
- Phone number
- Date of birth
- Social security number
- Insurance information
- Current problematic behaviors
- Mental health and addiction history
- List of medications
- Recent lab results
- Pertinent medical history
